The Michigan Anticoagulation Quality Improvement Initiative (MAQI2)
Anticoagulation Toolkit (Version 1.9) 2019
A toolkit for anticoagulation management was published by MAQI2 in 2017 and includes tools to assess stroke and bleeding risks, a comparison of the safety and efficacy of anticoagulants, patient characteristics that may influence drug selection, and a decision tree pathway for anticoagulant selection in nonvalvular atrial fibrillation (NVAF). Also included are dosing recommendations for patients transitioning between anticoagulants, a list of possible drug-drug interactions (DDIs) to consider, and patient education resources. Considerations for periprocedural management, including in patients undergoing elective cardioversion, and for bleeding management, including need for reversal, are provided. Recommendations for follow-up intervals and assessments are made.
